Participation

Thank you for your interest in our research study of a topical gel for papulopustular rosacea (rosacea with red bumps and pimples).
 
We would like to ask you to complete a screening questionnaire to see if you meet the criteria to participate in this trial.
 
Another purpose of this interview is to see if you want to hear about other research studies that will be done in the future; we are keeping a database of contact information to use for such future research purposes.

Information about you that you provide in this questionnaire will be kept as confidential as possible as required by law.  It is possible that the Food and Drug Administration, and other federal and state authorities, may inspect this record.
You will be asked to answer several questions. You may choose not to answer these questions. If you refuse to answer the questions or stop answering them at any time, there will be no penalty, and you will not lose any benefits to which you otherwise would be entitled.
 
If you would like to participate in this study, please fill out the information below. You must click the "Press to Request Online Survey" button to receive the email link for the participation survey. Once you enter and send your email address, you'll also need to press the continue button when prompted.

The risk to answering these questions is very small. This screening questionnaire is not designed to ask you for sensitive personal information, but it is possible that some people may feel uncomfortable answering certain questions online. If you qualify to take part in the study and are interested in taking part in this or future research studies, we will retain your name along with your answers to our survey and will record your contact information; this will be kept confidential. We will then contact you to schedule an appointment. We do not share our information with outside departments or research groups. There may be a small risk that people outside of the research team could accidentally learn this information. If you decide that you are not interested in the study, and do not want to be contacted regarding future studies, we will destroy the personal information that you have entered.
 
The benefit of taking part in this survey is that you will find out whether you can take part in this study, which may help us understand the effectiveness and safety of Ziana when used to treat papulopustular rosacea, and would provide more information to the sponsor of the study.
 
The benefit to you of being in our database is that you will hear about studies in the future in which you may be interested.
 
If you do not want to answer these questions, you can choose not to be a part of the study.
